Crest Cloud is the offer: Crest runs, monitors and updates iDoc, and bills per page or by the monthly plan. You install the SharePoint app and approve access.

A customer-managed deployment is a separate annual subscription with no per-page billing. That fee is quoted. The 30-day trial includes 200 pages.
